Supportive Home Care Services for Individuals with Disabilities
Support in their residential setting which promotes independence. Offering assistance in the areas needed such as grocery shopping, social activity organization, transportation, meal planning, health and wellness, personal and community safety and much more.
Typically these services are offered when an individual has made the transition from a caregiver's residence to a residence of their own.
The hours of support provided each day/week/month vary for each individual. A program/schedule is established according to each individuals needs. (Needs are determined based on collaboration from Parents/Guardians, the individual, Case/Care manager, and others).
The goal of this program in general is that each individual will become more participatory in their everyday life and activities.
Specializes in Teaching Daily Living Skills to Individuals with Disabilities
Cooking, Cleaning, Laundry, Budgeting, Socialization Skills, Communication, Driver’s testing preparation, Personal and Community Safety, Time Management and more.
The goal of this program is to enhance or increase the skills needed to lead a more independent lifestyle. These skills could be used in the individual's own residence or it could be used in their current home.
Rehabilitation and Recovery Programming: (Shawano County Only)
Offer services through Comprehensive Community Services "CCS" of Shawano County.
Work with a diverse population to aid in their rehabilitation and recovery.
Programming to assist individuals with Grocery Shopping, Meal Planning, Health and Wellness, Community Integration, Personal and Community Safety, and much more.
Services which promote, encourage and teach healthy living for both the individual and their families.
Services are tailored to meet the unique needs of each individual (and their families).
Daily Living Skills Training and Supportive Home Care Services
Opportunities to participate in group training based on topics such as Budgeting, Parenting, Healthy Relationships, and more.
